WebSurvey@UW - Creating a Survey


This document describes the basic steps that you will need to follow to create a survey.

Create a survey:

Once you have logged into the WebSurvey@UW application and have chosen the appropriate Group Account (if you have access to more than one account). You will be taken directly to your “My Surveys” listing. If this is your first survey, there will be no surveys listed.

  1. To create a new survey, click on the New Survey icon NewSurveyIconin the tool bar, or on the Create New Survey Button CreateNewSurvey in the upper right hand corner of the screen.

  3. Enter a Title for your Survey.
  4. Click the “From Scratch” button.
  5. Click “Save” button save.
  6. You will be taken to the Modify Survey screen where, you will enter the questions and formatting items for your survey. You can choose to create a new question, copy an existing item (from the current survey, or any other survey that you have created in this Group Account), or copy an existing item from any libraries associated with this Group Account. We will create a new question for this demonstration.

  7. Click on the insert button insert to begin adding questions to your survey.

  9. Click on the dropdown menu to select the type of item that you would like to add.
  10. No matter which question type you choose, there are several sections that you will need to complete. For this demonstration we will be using “Choice – Multiple Answers (Checkboxes) item type. Anywhere you see a red asterisk * is a required field and the appropriate information must be entered.

    The first section requests information about the question itself.

  11. Enter the question text. This will be what your survey takers will see.
  12. Enter the question alias. Your users will not see this, but we recommend that you enter this if you will be analyzing your results with Excel or SPSS. Question aliases are typically the question numbers represented by Q1, Q1A, Q1B etc., or text that will help you remember the question like “name”.
  13. Enter any subtext. This is information that will display directly below the question text and can be used for clarification, or to provide additional instructions to the survey takers.
  14. The Options section allows you to select whether the question is required for the survey taker, if the answers are to be displayed in the specific order or a random order, how the answers are aligned and if there are any minimum or maximum response limits.

  15. Select the appropriate options for your question. You may always change these before the survey is deployed.
  16. The final section allows you to input the answers for the question selection. You can choose from optional answer groups or you can enter your own answers. In this demonstration we are entering our own answers.

  17. Enter your answer choices.
  18. Choose if you want to have an additional text box for “Other” answers.
  19. Once you have entered all of your question information.

  20. Click the Save button save to add this question to your survey.

    Once you have entered your question, you will be presented with several design elements including page properties and question order options.

    Continue to enter your items. You can do them all at once, or come and go as you please. Once you have completed entering the questions,

  22. Click on the Done button done on the bottom right of the list of questions, and you will be taken back to your My Surveys page, where the survey you just created is now listed.

MySurveys

Once you have created your survey and at any point during the design process, you will need to select your survey options. To select your survey options, click on the Options button OptionsIcon to the right of the survey title. Survey options include the following:

General Information

Respondent Access Level

Survey Completion Properties

Navigation Properties

Hidden Fields

IP Address Validation

Numbering Properties

Other Administrative Properties

Administrator Properties
