WebSurvey@UW - Creating an Account

To create a WebSurvey@UW Group Account, please go to the online DoIT Tech Store to place your order.

You will be required to supply the following information:

• Primary Administrator – UW Madison NetID
• Account Type you are requesting
• Single Survey, 5000 respondents (41980) - $50
• Multiple Surveys, 5000 respondents total for all surveys (41981) - $150
• Single Survey, 10,000 respondents (41982) -$100
• Multiple Surveys, 10,000 respondents total for all surveys (41983) - $300
• Requisition/purchasing information

Once you have provided this information and submitted the request, your WebSurvey@UW account will be available in approximately one hour. You can then login at http://websurvey.wisc.edu.
